Box 32: service facility location

The service location is where you held an appointment with a client. To add your service location to claim forms, you need to go to My Account > Settings > Billing and Services > Insurance. There, check the Service Facility Location (Box 32) option and then click Save.Adding your service location on claim forms in SimplePractice

Selecting this option will automatically populate box 32 in your claim forms with the address of the location you assigned to this client when you scheduled their appointment.
