How to bill insurance under your supervisor's credentials

If you’re provisionally licensed under your supervisor and you want to offer appointments that can be covered by your client’s insurance, this is called incident-to billing and can be done legally.

Incident-to billing allows supervisees to hold appointments without their supervisor in the room, as long as the supervisor is facilitating by being present on location during the appointment and reviewing their notes after.

Important: We recommend reaching out to the insurance payers that you plan on billing to determine your eligibility for this type of billing and the requirements you must meet.

Please keep in mind that these requirements will vary by state and by payer, and it’ll be your responsibility to make sure that you’re submitting claims in the correct format. Certain regulatory bodies require that box 24J lists the supervisee's name with the supervisor's credentials, while others require that the supervisor's name and credentials are listed.

Once you’ve confirmed that you’ve met all requirements to perform incident-to billing and you know what format your claims will require, you can update the settings in your SimplePractice account so that the appropriate information will auto-populate. In this guide, we’ll cover:

To make sure that the supervisee's name and the supervisor's credentials populate onto your claims and superbills, the supervisee can start by going to Settings > My Profile > Clinical.

From this page, the supervisee can enter their supervisor's license, NPI number, and taxonomy code. The supervisee can also check the I'm pre-licensed under supervision box and select their Supervisor from the drop-down menu.

Tip: If you have your own license number but not your own NPI, we recommend listing your license number and entering your supervisor’s NPI and taxonomy code.

Once you’ve saved these settings, you’ll see that the supervisee's name populates with the supervisor’s credentials as the rendering provider in box 24j on claims.

Note: If you’re the Account Owner or a Practice Manager, you can update these credentials for another clinician in your practice by going to Settings > Team Members, clicking the name of the appropriate team member, and updating their Clinician Information.

To make sure that the supervisor's name and credentials populate onto your claims and superbills, navigate to Settings > My Profile > Clinical.

From this page, the supervisee will want to check the I'm pre-licensed under supervision box and select their Supervisor from the drop-down menu. Leaving the NPI Number field blank will trigger the system to pull the name and credentials from the supervisor's account settings.

Once you’ve saved these settings, you’ll see that the supervisor's name and credentials populate as the rendering provider in box 24J of claims.
