Entering "Paper Intakes"
Friends, I have some older clients who do not work online. If this helps, I recent entered my intake form as an assessment and filled it out for them using this tool. I can print it and send it to them, or we can work on it together in session. They don't have to touch a computer.

Here is the trail:
Settings> Notes and Forms (under the heading 'Documents')>Create New Template>
When you create a new template make sure to select the drop down and assign this template to 'Assessments'.
Enter the questions you use for your intake and then save.
It worked so well for me today and is something you can return to again and again.
