I've had a certain claim rejected twice for two different reasons. The first one was due to the Dx code. Now I'm receiving this error message and I haven't come across a solution.
Acknowledgement/Returned as unprocessable claim-The claim/encounter has been rejected and has not been entered into the adjudication system.
Missing or invalid information. (A3 - 21)
I am experiencing the same thing with Texas Children's Health Plan. I have seen MANY others with this same issue. One person had BCBS and they had to clear their information from box 33 because they didn't want it matching the facility information. Which is weird. But there was no follow up so I have no idea if that even worked. And I have this issue with a totally different payer. There are countless submissions and then someone just comes along and says they put in a ticket but no information for people who are coming on here for answers...
