Red Flags Rule - policy re: photo identification

I am looking for a sample policy specifically related to the Red Flags Rule adopted in 2010 - collecting photo identification to prevent and mitigate medical fraud.

Does anyone have a sample that I could look at to make sure I'm including all the pertinent information?
