    Hi there Kelley,

    I just received a payment for a claim that had this error initially. All I did was take out a hyphen in the client's last name on the claim and it successfully went through less than 24 hours later. This was just my experience with this, not sure if this applies to you, but wanted to share.

    It has to do with the demographic information. Ours was the DOB didn't match the Insurance DOB.

    Hi all,

     

    It will related to DOB which is incorrect or else 99% the error is, there will be DOB miss match between Patient DOB and Insured DOB in 11a. There might be two different DOB entered in the box if Patient is the insured.

    Thank yo u for this! I am dyslexic, miss over things, and did not realize DOB was on there twice! I didn't correct both.
