disappearing appointments

It has come to our attention that we are having some recent issues with our scheduling system in SP, which has led some new clients showing up for appointments when they have received notification for an appointment. However, these clients are not on the therapists schedules for the times their confirmation text states. SP does not have any record of scheduling/cancellations nor are these clients showing up on the clinicians calendars.

  This has happened to me a few times recently, too, and has caused me to lose clients. Any insight into how to fix this will be much appreciated. Thanks in advance.
