Print Demographics form

I am able to print out all of the intake documents, except for the demographics form. When I click on it, it brings me to the Support page in terms of what to do before sharing it. I have an in person. Client would like to fill out the paperwork in person, but I don't seem to be able to print out the demographics form. Please help. Thank you!
