Hi! Does anyone have a sample "supervision contract"? Open Path Collective is asking for one for my pre licensed clinicians.

  In order for pre-licensed clinicians to practice in New Jersey, they MUST be supervised, and the supervisor must fill out a plan of supervision. These plans are provided by the state's governing body for therapists, and are available for download. Can you find something similar from your own state's website?
