I received this error message and not sure how to resolve this. 

 

Acknowledgement/Rejected for Invalid Information - The claim/encounter has invalid information as specified in the Status details and has been rejected. ACK,REJCTD FOR INVAL INFO-CLAIM,ENCNTR HAS INVAL INFO AS SPCFI ED IN STATUS DTAILS AND BEEN REJCTD. * PAYOR CODE: A7:488 INVALID DATA: PAYOR MESSAGE: ACK,REJCTD FOR INVAL INFO-CLAIM,ENCNTR HAS INVAL INFO AS SPCFI ED IN STATUS DTAILS AND BEEN REJCTD.:DIAGNOSIS CODE-S FOR THE S ERVICES RENDERED. (A7 - 21)

  why are the rejection reasons in code and not just communicated in social worker language. just tell me what exactly i did wrong?
