cannot resend PHQ & GAD to client for reassessment - HELP!!!

I can't resend a blank version of the GAD or PHQ to the client for reassessment. Please help!!!

  There are several tools I use to reassess, including these, the ISI and Epworth. Is there a way to send at each visit or to choose specific forms to resend?
