EMDT conceptualization templates

I am in need of any templates pertaining to EMDR. Does anyone have any ideas? I have an EMDR progress note that apparently came with my simple practice, but I know people who have more EMDR templates. I don't feel confident making one from scratch. Does anyone have any ideas?
