Paper intakes for low-tech clients

For clients who do not use or have access to the internet, is there a way to have them complete intake forms on paper and later scan them into the chart? Or, is there a way to pull of their intake forms on my computer and ask the questions in an interview?

 

This question was asked by someone else a year ago but not answered.

  For clients that don't have internet or are not computer savvy, I will mail them the intake forms and they bring them to their first session. I realize this may not be possible if you are solely telehealth. 

  I scan the forms into the chart.
