Consent for treat minor

Trying to find the form to treat a minor and cannot find it anywhere. Just wasted a half an hour trying to Google and look through the questions and answers. NONE of the answers are helpful. Why is the form not listed already? 

Who has time to keep digging around trying to find simple forms? Lots of Simple Practice clients must work with minors. This isn't rocket science. I have so much going on it's very very stressful in and of itself, but to spend so much time simply trying to add a consent form for a new minor client is overwhelming. I cannot submit the paperwork to the parent now after days of trying because I do not have that form at my disposal. I've even wasted a lot of time trying to Google online forms. NONE WORK EITHER!! They all want approval to work with kids for medical issues.

I have already followed advice on the forums. None of it worked. It was also super long and clumsy.

How hard is it to add a consent to treat a minor as part of the standard list? Most therapists are not good with technology so that adds another additional layer and burden to overcome. Who has time or energy for this? Customer service also does not get back to people in a timly manner and it's all email. SO unhelpful. 

I am seriously considering changing to a different format. It doesn't need to be this difficult. Isn't life hard enough?

Very, very disappointed.
