SP with Target Case Management and Community Support

Hello,

I am looking for a group practice that uses SP for their Medicaid Target Case Management and Community Support Associate positions.  I would like to connect to someone in this community to gain information on how they utilize SP for TCM and CSA needs.   Note-writing and template?   I was explained by SP specialist to edit the existing template for TCM use, but it would help greatly if I could see how existing group practice uses the SP for their TCM and CSA use.   TIPS be great as well.
