saved notes have vanished from a client file

As I was billing and calculating a former employee's last paycheck I noticed session notes missing. However, as the supervisor I had to sign off on the former employee's session notes because they forgot to do so before being exited out of the system. Therefore, I know the notes for the 6 sessions were complete. They were initially appearing as duplicates under each session and now nothing. Are they located anywhere else? 
How can this be fixed and prevented from happening again?
