TELE-HEALTH client: How to send Patient Safety Plan TEMPLATE via Client Portal for them to complete
- I just had a tele-health session w/ a client (w/ suicidal ideation) who lives 2 hours away.
- We discussed each question in the Patient Safety Plan during this first session.
- I said I'd send the TEMPLATE via the Client Portal so she could enter the information IN REAL TIME, save the Safety Plan on her computer, and upload the file to the Portal.
- The Community Post suggestions on this subject from 4+ years ago didn't help.
- Is this possible, or is my only option to: 1) send a PDF of the template; 2) ask her to print out a copy (doesn't own a printer); 3) fill out the form by hand; 4) find a scanner or take photos of the form w/ her phone & 5) save the file to her computer & 6) upload the file via the Client Portal?
- I need to get this to her ASAP. Thanks in advance for your suggestions.

Hi Sharon,
I will suggest you create a Patient Safety Plan template in your template library. Then send it as a document for the client to sign, just as you send other intake documents. Once client is done, you will have a copy and can email it to client as well if they are unable to save it.
