Release of Information Form
The ROI form I have used for years has a place for me to write in the information about each doctor/agency/provider that I wish to communicate with (on separate lines). I fill it out for the client, because this is often confusing for clients. I hand it to them (or email it) and then they sign one form, for all the places I am coordinating treatment with. Is there way to create a document where I can do this in Simple Practice? I don't want to send them a blank form that they fill out for each individual agency/doctor separately. Plus, if I have to do this, I will have to talk many of them through how to fill out the form each time and that is a lot more time consuming. I would like to send them a form and say "I am sending you this release form. Click the button to sign it."
