    A nice idea--it certainly will be helpful for those of us on Medi-Cal (California's version of Medicaid).  Why not create the template in SP and upload the images of your idea?  That way we can all see what it will look like.

    I want to see the form you created as it appears as a Simple Practice template, in other words, I want to see screenshots of the form you have already designed.  It's one thing to suggest the content of the form you want to be created and another thing altogether to use the template design tool to create the form you are suggesting.

    Hi Ryan, thanks for being plain about what you want. These aren't suggestions.  These are forms I created.  I selected the text in the template, copied it, and pasted it to here.  When I pasted it, the box graphics were displayed.  Then when I loaded the page, they were gone.  So not really sure what I can do for you except encourage you to advocate to SP to allow these templates to be downloaded and uploaded. 

    When I get the time I will screenshot for you, there's a lot going on though so please be patient.

    how do I proof timely filing on a claim that I mailed claim.

    I dropped claim (hcfa) and mailed to insurance?.
