    Great stuff, Ryan! I'd love to see this as a new template! I use the one in there already because I'm too busy with the other new and crazy things that go along with opening a newer private practice! The template in there currently is sorely lacking and this one gathers much more information! Love it and I hope others vote for it! Hope to see it in SP soon! Thank you for creating it!

    I'll vote for you.  I already have one that I use but I don't know how to do screenshots to add it here,  I agree the one in the system is currently lacking. The reason I went with SP is so I can make my own templates.

    HI Ryan- If I put it in as a PDF is the client able to fill it out and return it to me electronically or do they just bring it to the first session?

    Agree for template!!! although I would still add some on family and family of origin-just simple on who is everyone and what are the relationships like in one or two sentences. Agree that it's something I keep meaning to do and never getting to it!

    Thanks so much, I am in the processing of trying to create my own template...when i use the one on simple practice for my intake I find myself asking a ton of questions that I wouldn't normally have to ask.

    Something similar is available in the template library with the title "Intake Questionnaire - Long Version."  The PDF version in the included link above is the most current and complete.

    We've included this form in our Template Library. You can go to My Account > Settings > Notes and Forms > Template Library and hit the +Add link next to the "Intake Questionnaire - Long Version" to use it.

    Ryan - thanks for sharing this!  As a sex therapist who works with diverse sexuality populations, I love that you are including consensual non-monogamy and kink as options for people to check.  I have them on my forms, and its great to see them elsewhere.
