Client Safety Plan Template
CompletedThis is a suicide prevention tool. It is preferable that the form is completed with the client present and printed out for the client to carry in a pocket, purse, or wallet (when printing, reduce the document to 51% and print 2-sided). However, you may find an application in which the client could receive it via the Client Portal, then discussed in a session. I see this as documented evidence of a good-faith effort on the clinician's part to provide support to a client with suicidal ideations. I just attach it to the client's EHR and maybe add a chart note making reference to it. I also print out a copy for the client to keep so they have an action plan in the event of suicidal ideations. The point of this safety plan is to document evidence of a good faith effort to prevent a suicide in the event of a tragedy.
