Client Safety Plan Template

This is a suicide prevention tool.  It is preferable that the form is completed with the client present and printed out for the client to carry in a pocket, purse, or wallet (when printing, reduce the document to 51% and print 2-sided).  However, you may find an application in which the client could receive it via the Client Portal, then discussed in a session.  I see this as documented evidence of a good-faith effort on the clinician's part to provide support to a client with suicidal ideations.     I like your template.  Nice for the client and for documentation.  What do you add to your note to indicate this happened in session or do you just attach it?

    I just attach it to the client's EHR and maybe add a chart note making reference to it.  I also print out a copy for the client to keep so they have an action plan in the event of suicidal ideations.  The point of this safety plan is to document evidence of a good faith effort to prevent a suicide in the event of a tragedy.

    How do I attach this to the client's EHR? I see how to preview it and duplicate or revise it, but not how to attach the form to a particular chart.

    Truthfully, Eric, in order to attach it to the client's EHR, it must be sent to the client's portal.  I realize that this is not possible in an office setting unless you have a tablet and the client can log into the portal on the tablet.

    How to you send this to the ct's portal.  I see the template, but it seems to only allow me to add it to the "progress notes" section and won't allow me to add it anywhere else?

    In the client's Progress Notes Overview page, on the right, scroll down to the Shared Documents section and click Share New.  You should be able to select the template from the resultant window that displays.
