How do you complete a release of information and then share with your client?

    Hi there, Vanessa. 

    I hope your Tuesday has been swell so far. I can understand wanting to get your forms set up perfectly, so no worries, I'd be happy to help. 

    If you haven't already, you can add your Release of Information (ROI) form to your notes and forms library by going to My Account > Settings > Notes and Forms > Template Library > +Add

    Once this has been added, you can send your ROI to your client by navigating to your client's Overview Page > Share New > Select the ROI > Send Message. Once your client has filled out this form in the Client Portal, it will automatically store directly in their account for you to review.

    I believe Vanessa meant that she is having difficulty understanding how to add the person's contact information to which the client is consenting to release information herself,and then sharing that document with the client so they will only have to sign electronically.

    That is actually my question, as well. I would like to prepare the release for the client after having talked to the client. This would also be helpful in working with children or teens when the parent may not be present at the session, but has access to the patient portal.

    I am looking for the same information.  I cannot seem to edit who the release is for, nor is my client able to.  ???

    I have had the same problem...the clinician needs to be able to fill out the form with the details and send it to the client for signature and allow it to be returned to us.

    I have the same question.  AND in addition is this release valid in ALL states, esp for TITLE 42 information (substance use, HIV) etc. Not sure who to call?

    Hi everyone, 

    I apologize for any confusion on my end. I'd be happy to help out here. 

    If I'm understanding correctly (and please let me know if I'm not), it sounds like you are looking to edit two sections of the ROI: the 'To/From' section, and the 'Client's Name' section. 



    If this is what you are referring to, I know exactly the fix. This section is meant to be a "Paragraph Text" section, so you can edit this to your liking.

    As you can see from my example below, I have added my practice name below to indicate the information can be released to and from my practice: 



    If you would like, you can remove the section altogether by selecting the blue trash can to the right of the question. 

     

    As for the Client's Name, we are unable to pre-populate information in this section, so we give the client the ability to enter their name in. If you are wanting to add a client's name manually to their ROI prior to sending it to them, I recommend creating a duplicate ROI by navigating to My Account > Settings  >Notes and Forms.

    Go to the template library.  Scroll down to Release of information and click +Add

    Now you'll have a duplicate ROI. You can do this as many times as you need. 

    Select it to retitle it for this client in particular. 



    I recommend adding client initials to this particular ROI form's title that is specific to the client, so you can know who it pertains to and who to send it to.

    Once you have sent this to your client with the 'Shared Document' option on their Overview Page, I suggest going back to your Notes and Forms settings and deleting the form (so you don't have too many forms laying around!).

    This does make sense. This is what I have started doing. I have several releases in my template library and can choose based upon the entity to be released. I think the ideal situation would be a ROI in which I could complete all the fields needed except for the final signature. In F2F practice, I would often complete the paper released with the client, review the information with them, then ask for a signature and sign as a witness.

    Thank you for your help Melinda, this answers my question, somewhat.  However, I agree with Leslie.  Also, none of my clients (nor I thus far) have been able to edit the form to input who the release is for (which I now have an idea of how I can do this thank you, but not the client).  This seems so cumbersome and if we have to edit a form in the general Notes and Forms section to include specific information for a specific client, this seems risky from a HIPAA standpoint...  Just would be nice to be able to edit this more easily in a specific manner for the client only.

    Is there a way for the Clinician to also sign the release electronically after the client does? Can the system send the release back to me after they have completed it so I can click a box to sign electronically as well.

    Is there a way for the client to complete the ROI and sign at the office? I like to review ROI's with my clients so they understand what they're signing.

    I keep having issues with clients completing an ROI and then it does not show up as completed on my end. Why is this? Anyone else having difficulty with that?

    This ROI is a mess!!!!!   We should be able to fill it in as the provider and simply have the client sign it. If filled out incorrectly the release is invalid.   Please make this form available in a drop down menu within the session so the therapist can open it and complete it IN SESSION with the client present and explain the form and have client sign.

    how do we put  complete the form will all the correct information then send it to the client to sign will all the correct information filled out? ROI if we duplicate it then all the information I preloaded is gone when I reopen it to preview to send to the client.
