Changing a Client's Diagnosis...
AnsweredHow do I change a diagnosis in a client's chart? They have been seen as one half of a couple for years by another clinician in our practice and will now be seeing myself for individual therapy. Thank you!

Hi Mary! To change a client's diagnosis, simply navigate to the original diagnosis that was entered on the client's Overview tab, and click "edit" when hovering your mouse over the diagnosis and treatment plan. Then, change the diagnosis, and update the timestamp of the diagnosis if needed.

Hi Mary, to create a new Diagnosis and Treatment plan, click the +Create button on the client's Overview page. Then, remove the previous diagnosis, and enter the new diagnosis. The old diagnosis will still be retained, but the new one will appear on any future billing documents and in progress notes.

Hi Jennifer! While I don't personally bill insurance, I'm part of the SimplePractice Customer Success team, and I'd be happy to help. If you'd prefer not to show multiple diagnosis codes on the CMS-1500, you can go to My Account > Settings > Billing and Services > Insurance and uncheck "Include multiple Diagnosis Codes" to show only the most recent diagnosis.

Hi Jennifer, in that case, I would recommend removing the previous diagnosis and then adding a Chart Note to indicate the client's previous diagnosis. You can backdate the Chart Note to the date of the previous diagnosis to ensure that the date and time of the diagnosis is appropriately reflected on the client's chart, without pulling the previous diagnosis into claims.

To add a new diagnosis to a client's profile:
- Go to the client's Overview page.
- Click +Create New > Diagnosis & Treatment Plan, as shown below.

I am also having this problem. I want to retain the old diagnosis in the record, add/update a new diagnosis, and reflect the new diagnosis on the SuperBill. For whatever reason, the old diagnosis still appears on the SuperBill.
This seems like a relatively easy fix, from a programming standpoint, however, I'm not sure if Simple Practice is aware or concerned about this glitch seeing that the problem persists after four years, looking at the start of this thread. Can anyone help with this?

Hi all. I believe I solved the issue!!
To end an old diagnosis from showing up on a superbill, you need to create a new blank DX & treatment plan. This means DON'T enter any new diagnosis. Instead, leave it blank and click on "skip to treatment plan". You can leave that part blank, too, or enter notes as to why you are ending the previous diagnosis in the text box. Double-check the date and time you submit the blank DX & treatment plan, then create a new Dx & treatment plan with the updated diagnosis and treatment plan. Just make sure the date and time are a few minutes after the blank one (mine was two minutes later). Now, all documents and superbills dated AFTER the last DX & Treatment plan you created will print with the updated diagnosis. Plus all the older notes and superbills will maintain the old diagnosis and not include the new one.
