Here is my Practice Policies document for your review.  Some would incorporate this information into their Informed Consent document, but I decided to keep them separate out of consideration for brevity.  If the Informed Consent is too long, clients won't read it and those who do will skim it at best.  Besides, how I conduct therapy is distinct from how I run my business.

    First, you have to download the PDF and edit it to your liking.  Then copy and paste the content to the Template Editor in My Notes in Forms (My Account > Settings > Notes and Forms).

    You can get it to appear in the document selection window when you add a new client.  Go to My Account > Settings > Notes and Forms.  Then click the checkbox to the left of the template you just created so it will appear in the new client window.
