Hi Gay, to create a Mental Status Exam or Treatment Plan, click +Create > Mental Status Exam or Diagnosis and Treatment Plan in the client's chart from the Overview page. These assessments are not attached to a date of service and can be completed whether the client has an appointment scheduled or not.
To use the Initial Clinical Mental Assessment and Treatment Plan Progress Note template, first, add it to your available Note and Form templates from the Template Library by clicking the +Add button under My Account > Settings > Notes and Forms > Template Library. Then, when the client has a scheduled appointment, click +Add Progress Note on the client's chart under Overview. Finally, select the Progress Note template you'd like to use from the drop-down menu on the Progress Note.

Hi,
When I click "create" it only gives me options for creating a client or an appointment. I don't see a treatment plan or assessment. When I try to add an appointment it only gives me options for individual, group etc. but not an option for a MH assessment. Where can I find the treatment plan form and assessment form (not just the MHS Exam but a full assessment form?) I know it's probably hiding right in front of my face,

I am also having trouble completing assessment forms to add to the client EHR. I can view and edit the assessment forms in Documents/Templates, but how can I fill them out on the Client's page? This is something the clinician does, so I can't send it via email for the client to complete. Please advise. Thanks!

Elizabeth, you can use a Progress Note template - including templates for assessments- when the client has an appointment. Just click "Add Progress Note" on the client's chart and select your assessment from the drop-down template menu. If you'd like to do the assessment with a note as well, you may want to include a note field in your assessment template.

Hi Laurie,
You can share forms with your clients through the Client Portal. On a client's page, click the Share button at the top right corner. This will pop up the same window that appears for sending documents to new clients. The assessment should show up in the list, and you'll be able to check its checkbox and click "Send message" to send the form to the client.
If you need additional assistance with forms, the client portal, or any other features of SimplePractice, I recommend using the red Help button in the lower right corner, and choosing the email option for a support request.

I would like to do my own MH History Form in an intake appointment. It is already saved in intake forms and already added to available forms. I regularly send this form to new clients, but when a client doesn't complete it before the first session I would like to be able to do it during the session with the client. Is this possible? This form does not show up in progress note or in MSE or Treatment Plan.

Sure thing, Reaca. What you'll want to do is duplicate that form by going to My Account > Settings > Notes and Forms, click the Duplicate button, and then convert the copy to a Progress Note template by clicking on its name, then selecting "For: Progress Notes" in the drop-down menu at the top. Then it'll be available for you to use as a Progress Note template.

Thank you, Gillian. I was able to follow your steps and the form was available in the Progress Notes section. But when I go to a client's record, add note, and do the progress note drop down the form I added is no longer there. I then go back to Notes and Forms again and the form I added is no longer there (yes, I saved it).
I tried this process several times; and logged out/back in. It does not save.
Please advise?
