  I use this form all the time! I appreciate how it covers a wide range of symptoms, and can spark exploration of symptoms the client may have been hesitant to share.

  Please please please add this to the templates. It is indicated on one of the intakes as a measure ( you can click on it in the Initial Intake Assessment form) yet it is not available as a template ( ACES is)

  I would love having it, but I'm pretty sure the cross-cutting measures are copyrighted and there is a specific clauses listed in the DSM 5 about no permission given for any kinds of digital or app use of content.

  When it first came out, I researched building an app myself for diagnostic info and the cross cutting measures and then I came across that copyright info... And said oh well...

  Anyways, I'm not sure SP can do anything about that.

  You can always do it by hand, but that is a lot of work.
