I'm not sure about submitting through simple practice, but you should be able to hit "create claim" in Simple Practice and then download the form (with background) from there. Then you will have a PDF of the claim which you can upload to Availity.

Thanks but I found the correct one to bill through Simple Practice.

Hi Mary, we have a connection with New Hampshire Anthem Blue Cross Blue Shield under Payer ID NHBLS.

I do want to advise that this payer is picky about where claims are submitted. Blue Cross Blue Shield plans should always be filed to your local Blue Cross Blue Shield payer. This rule of thumb also applies to client's with an out-of-state Blue Cross Blue Shield plan. Once you submit the claims to local payer, the payer's internal system will re-route your claims for processing.

My rejected claim states billing provider not on file?  Can you suggest what I should do next?  I am an anthem provider and I am registered with Availity.  Thank you

Hi. I'm in a similar position. I have Anthem clients in Oregon and there's no option for Anthem Oregon. I submitted to Anthem CA (which seemed like the correct code) and I got a rejected EOB saying I needed to submit with my local chapter. I resubmitted to Blue Cross Oregon and that didn't work either. Is there a code for Anthem Oregon not listed in the scroll down? Would love your help please.
