Patient Face Sheet for Referrals
What do you use when making referrals to other clinicians? Would love to have a form within the system to populate the client demographics, and maybe a few other check boxes or paragraph boxes to create a one-sheet referral form. I am an LPC, and find myself needing to give referral forms when I recommend a psychiatrist or psychologist to someone.
Any templates you all find useful or do you just create as you go?

Hey Linda, I ended up using the function on the client's main page: Create New > Diagnosis & Treatment Plan.
I offer my general diagnosis, then used the "Basic" template. It gives you a place to describe goals and objectives. I used that to outline why I was referring. Then when you download it to fax or send, it has the client info at the top.

Hi everyone who replied to this thread about having a "face sheet" option in SimplePractice (an easy way to have one document of info when we refer to outside clinicians).
SP now has a feedback site that collects votes and comments on issues that are posted. Here is the link. If anyone wants to vote on this handy option, I think it would be great to have!

Hi - Thank you all for making such great suggestions on how to create a facesheet from your client files. I'm chiming in with a suggestion of my own, here are my instructions on how to download your client's Overview page to include all the contents of a facesheet.
1. Go to your client's Overview page, select the Diagnosis & Treatment Plans display option and make sure you're in the Excerpt view (two horizontal dashes instead of three)
2. Next, use the arrows to the far right of each box in your client's file to include or disclude the information in each box.
For example, you'll want the arrow pointing upward to include the CLIENT INFO box, INSURANCE INFO box, CONTACTS box, and UPCOMING APPOINTMENTS box. You can disinclude a box by making sure it's corresponding arrow is turned downward.
3. Now Print as a PDF and deselect, or uncheck the boxes to include "Backgrounds" and "Headers and Footers."

Have there been any updates to providing a face sheet with demographic and insurance information for referring to other clinicians? The suggestion above doesn't really provide what I need. I would rather the printout not look like a computer screen, but an actual document with all of the information needed. Thanks!

I went to that link and voted for this upgrade. It's critical that we get a printable face sheet for multiple reasons but also to make referrals to outside providers.
