    How do you take this form and make it so the patient has to "click to agree with it" (so we can have some sort of electronic signature

    Hi Idalia, under My Account > Settings > Client Portal > Shared Documents and Files, add the Consent Form. It will automatically have a checkbox that takes a legally binding eSignature when you send it to a client through the Portal.

    I have been using an outside consent form for my clients, but have recently turned on the client portal. How can I send the telehealth consent form to existing clients that I have been seeing prior to me enabling of the client portal?

    Hi Meghan, happy to help with this. You can send Consent Forms and Intake Forms to existing clients via the Share button on each client's chart. If the client doesn't already have Portal access, sending a Consent Form or Intake Form to them via the Share button will also send them an invite to the Portal and instructions on logging in.

    Hi Michelle - Yes, there is! To print a blank Telehealth Consent Form, go to Settings > Client Portal > Shared Documents and Files, find the form, click on the eye icon to preview it, then click the Print button to the top right corner of the preview page.

    Hi Gail, please refer to my answer above for more information on how to print out blank Consent Documents. If you'd like to print out Consent Documents that your client has already signed, go to their client file and find the SHARED WITH CLIENT box to the bottom right and click on any signed form you've sent them, then click the print icon to print it!

    Hi Rachel, yes! You can create the Consent Form as a Note and Form Template under Settings > Notes and Forms. To add text, add a Paragraph Text form element. Clients can enter information with Short Answer or Long Answer form elements. You can add an eSignature checkbox at the bottom to obtain client's eSignatures as well.
