Hi Jason, I'd be happy to help with your question. Currently, there is not a way to add your logo to templates that you've created in SimplePractice.

Why was I asked to upload it, if I can't access it for use? Thanks, Sara

Sara, your logo is displayed on invoices and statements.

Hi there! To add your logo to invoices, upload it under My Account > Settings > Documents.

I would like to request the size of a company logo to be increased, The size is currently restricted and not clear on our documents.

Hi Tim, often doing some adjusting with the logo file before uploading can help it appear larger on invoices and other billing documents. For example, if there's a white border on the logo, cropping out the border will increase the logo size. Our Success team would be happy to assist with this - just reach out through the red Get Help bubble for support.

I would like to request that you make it possible to add logos to notes and forms.  I often have to print copies of the forms for customers who are not tech savvy.

Hi!  I am still in my trial period and had a couple questions- I notice that the "logo" portion always prints to right side of page.  Is there a way to print in center of page, like a stationary would have it?  Mine is made to fit that part of page more.

I would like to include in my intake forms a Medications List for my clients to fill out. How can I create a table for them to list medication names, dosages, prescribing physician's names, and reasons for taking medication. Or how can I upload a previously created Medications List in my list of intake forms? Thanks in advance : ]

    Hi Heidi,

There is not currently a way to edit where your logo appears on documents.

I encourage you to add this idea to our Ideas and Suggestions board. Posting on this board means that members of our Product Team, as well as all members of our Community, have transparent access to view, vote, and comment on your suggestion.

    Hi Tracy,

While there is not currently a way to add a table to an Intake form, you can select the Long Answer (Multiple-line text) option and provide guidance for the information the client should include.

If you have a moment, I also encourage you to add this idea, or vote on similar ideas, on our Ideas and Suggestions board

Hi. I'm trying to get my logo to show up on a consent form I created. I have checked the "show logo" box under documentation and it still is not showing up on the consent form.

    Hi Jennifer,

When you add a logo to your documents, it will only appear on printable versions of documents. It will not appear in the draft or unsigned versions.

I’ve also gone ahead and created a help request on your behalf so that a member of our Success team can take a look at your account. They’ll be reaching out to you with more information over email.
