Trying to clarify note templates

Can I move or copy one of my custom intake forms to the Progress notes forms of the Notes and Forms setting so that it is available to use an option when charting session progress notes?  Or do I need to redo the entire form?  Sometimes I complete the entire intake in session, the client does not fill this out via email.  I haven't found a way to access the intake forms to complete while I am in the client's chart.

    Hi Danielle,

    I'd be happy to help you with this. I just did a bit of testing, and there is indeed a way to do this. In the Notes and Forms section (My Account > Settings > Notes and Forms), you can click the [+] button to the right of the form's entry to duplicate it.

    Next, open the copy of the form you just created. At the top, there will be a dropdown menu that read "for Intake Forms". You can click this and change it to "for Progress Notes". When you save the form, you'll see that it's moved from the Intake Forms section to the Progress Notes session. The form will now be available to use for session progress notes.

    So if a patient doesn't fill out their paperwork before they come to the appointment and you want to have them fill it out in the office on a computer without accessing their email, the way you describe is the best and only way to pull them up?

    Hi Kim,

    This would be one way to allow a client to fill an intake form in-person. Another would be to print a copy of the intake form, and then have them fill it out by hand. You could then scan the intake form and add it to the client's Uploaded Files.

    The only other option would be to have the client sign in to their email account on site, follow the welcome link to the Client Portal, sign out of their email account, and then fill out the forms via the Client Portal.
