    Hi Danielle,

    I'd be happy to help you with this. I just did a bit of testing, and there is indeed a way to do this. In the Notes and Forms section (My Account > Settings > Notes and Forms), you can click the [+] button to the right of the form's entry to duplicate it.

    Next, open the copy of the form you just created. At the top, there will be a dropdown menu that read "for Intake Forms". You can click this and change it to "for Progress Notes". When you save the form, you'll see that it's moved from the Intake Forms section to the Progress Notes session. The form will now be available to use for session progress notes.

    So if a patient doesn't fill out their paperwork before they come to the appointment and you want to have them fill it out in the office on a computer without accessing their email, the way you describe is the best and only way to pull them up?

    Hi Kim,

    This would be one way to allow a client to fill an intake form in-person. Another would be to print a copy of the intake form, and then have them fill it out by hand. You could then scan the intake form and add it to the client's Uploaded Files.

    The only other option would be to have the client sign in to their email account on site, follow the welcome link to the Client Portal, sign out of their email account, and then fill out the forms via the Client Portal.

    Hi, the problem I have is I'd like to create multiple screening tools to use  in session and id like to fill them in while meeting with the client. is there a way to do this? the answer you gave above would mean I wouldn't also be able to attach a progress note or multiple questionnaires to one session.

    Hi Elizabeth,

    A session in SimplePractice can only be associated with one Progress Note and one Psychotherapy Note. If you have the client fill out questionnaires in-person, the questionnaires will appear as entries on the client's record on the same day as the session, but they will not be explicitly connected.

    However, sessions can also have files attached. It would involve several steps, but you'd be able to get the questionnaire filled on the Client Portal, then download the printable PDF version of the questionnaire, and then upload the PDF(s) as attachments to the progress note.
