    This is was I was told by Simple practice: The claims are rejecting due to the information that is listed in box 32 of the claim form matching the information that is listed in box 33 of the claim form. Certain payers will consider this to be redundant and cause for rejection. What is weird is we have not changed a thing and past claims have gone through just fine, so not sure why these ones suddenly rejected.

    Never had that problem before now all of a sudden my claims are being rejected.  Is this a software problem of SimplePractice??

    I have had three claims paid and now this message. I have the same information on all claims. So annoying. Will resubmit and see what happens.

    Same.  Mine are also Caresource.  Does anyone have a fix for this problem?

    I changed the address in block 33 to match block 32. They want a physical address, not a PO Box.

    I just received this code for CareSource clients as well and we have not changed anything with them. Our addresses do not match as the locations are different but have always worked for us in up until this week.

    I have had several claims pay for Caresource until this claim.  Our addresses match exactly in box 32 & 33.  Not sure how to fix this.  Should I just resubmit and hope for the best?

    I resubmitted all of ours that were just rejected. Over 2 years of billing w no problem until this week so hopefully just a system error. I will update once I know if they were accepted or not.

    Thank you Ashley!  I went ahead and resubmitted them.  I did notice the zip code in box 32 did not have the last 4 numbers but box 33 did, so I added that and will hope for the best.  I will also let you know if mine get accepted or not.

    I resubmitted leaving box 32 and 33 the same. Denied again. This time i resubmitted and deleted box 32. I will update if they pay. Again, all of these with this message have been caresource for me and only recently have i had an issue.

    Try making sure box 33 has all nine digits in zip code. This happened a while back with another payor and kind of fixed itself after a while.

    I am not sure from reading the comments. It does seem like a system problem since the above comments seem to have been rejected for different reasons. I add the last 4 numbers in the box 33 and resent. Will also update if this makes a difference. Or is the problem that there shouldn't be addresses in both boxes?

    I just got an answer for this and wanted to update here since there was no clear answer beforehand. This is directly from the Simple Practice email I received: 

    For some context, Box 32 (Service Facility Location) should not be included when the NPI in this box would match the NPI in Box 33, as the payer considers this duplicate information and will issue a rejection if present.
     
    The requirement of Box 32, the service facility location, is specific to each payer. Because most payers don’t require it, we recommend excluding it unless explicitly requested that it be included as it can cause claim rejections.
     
    In this case, I recommend resubmitting these claims without Box 32. Please do not manually remove the information from Box 32 on the rejected claim, as this will result in an additional rejection. This is because electronic claims carry back-end data that remains tied to the claim upon it's creation. Even after manually removing details from a claim, the back-end data may cause the claim to reject again.
     
    Instead, Box 32 can be removed from the claim by navigating to Settings > Billing and Services > Insurance, and then unchecking the “Service Facility Location (Box 32)” checkbox.

     

    We have several service locations and have been leaving box 32 with our organization NPI and billing to the rendering provider with the provider's NPI. I have resubmitted this way by looking at past claims for the payer. I am hoping this works in our case to keep our service location. I hope this helps
