Filling out intakes

Hi.  I have added my intake assessments to my notes and forms.  I even figured out how to send them to the client with the client portal.  However, if I was to want to fill it out on the computer with the client how do I do that?  Or do they have to fill it out on their own through the client portal?  What if they dont want a client portal?

    Hi Nicole,

    Client intake and consent documents are found in My Account > Settings > Client Portal > Shared Documents and Files

    Here you can:

    • Edit or create new consent documents.
    • Upload PDFs or read only files.
    • Manage intake forms, assessments, and questionnaires that your clients can fill out electronically in the Client Portal.
    • Edit or create intake documents by selecting the blue Go to Notes and Forms link.
    • View demographic and credit card forms.

    All of these documents are then available to send to your clients within the Client Portal for them to complete at their convenience.

    If you prefer to complete intake documents with your clients, you can send them the documents via the Client Portal, and then during a session, your client can log in with a web browser on a desktop or tablet. You can then complete intake together with the completed documents saving both in their Client Portal, and also in your SimplePractice account.

    For clients that do not wish to use the Client Portal, you have the option to print out blank documents from your Shared Documents and Files location as well as Notes and Forms. After these forms are completed, you can upload scanned copies into a client file with the upload button in the right corner of a client's overview page. For more detailed guides see:

    How does the client log into their portal if it's on my computer.. would they have to log into their email and then log into the portal?

    Is there a way that I can Download a blank Intake Questionnaire that I can fill out via goodreader and then upload it to client file?
