I would like to know how to add the assessment into the file without having to create a session to put it in.

I'm still in trial. Assuming that any forms filled out by the client or I fill out on behalf of the client gets linked to their record?  So many basics I need to learn first.

I'm wondering the same thing! We just completed an intake. The client completed the basic intake form on their portal, but we like to do a more in depth psychosocial assessment so we also did the biopsychosocial assessment....I just don't want this uploaded to the client portal so my client can see it. I want this as part of their record. How do I do this??

Hi everyone! Apologies for the delayed response. The Biopsychosocial Assessment in SimplePractice is a Progress Note template. To use a Progress Note template, first schedule an appointment on the Calendar for the client. Then, click +Add Progress Note on the appointment on the client's Overview page. Finally, select Biopsychosocial Assessment as your chosen Progress Note template. If you'd like, you can add fields to the Biopsychosocial Assessment for your Progress Note under My Account > Settings > Notes and Forms. Progress Notes are not shared with clients. Let me know if you have any additional questions about this, or anything else SimplePractice. I'm here to help.

    Hi Gillian and all,

Is there a way to "load previous note" from the initial biopsychosocial assessment I completed a year ago?  I currently am doing the steps listed by Gillian above for my initial assessment.  I like updating my assessments every year but don't want to recreate the wheel.  I am trying to find the simplest way to accomplish this?  Please advise and Thank you!

For most of my patients, their mental status does not change much from session to session. How do I load a prior mental status exam into a new appointment so that I don't have to keep doing it and recreating the entire medical status exam every week?

    Hi Samantha,

I see that you wrote in to our Customer Success team and they are working with you on your question.

    Hi Lawrence,

There is not a way to load a previous Mental Status Exam. However, here's how you can load a previous progress note:

    1. Go to the client's Overview page.
    2. Click on the Appointment heading for the appointment where you need to add a progress note.
    3. To the right of the Progress Note heading, click Load Previous Note, as shown below.
