    I wouldn't use a form that stated that. It is a violation of HIPAA to release PHI even to another healthcare provider without the client/patient consent. If you need to talk to a supervisor or consultant regarding a client simply don't use PHI (, When I was under supervision and when I am doing EMDR consultation we say things like "female, single, age 30". If the referring physician wants updates have the client sign a release.
