Templates for Intake & Consent Forms for Adolescents & Children Needed
AnsweredThere are no intake template or consent forms for adolescents/minors for behavioral health providers available. It would be very helpful to have a preloaded template for intake and consent forms available for children and adolescents. In Illiniois, children ages 12 and up have to sign the consent forms along with their parents. Thanks!

Agreed! It is shocking that Simple Practice does not have an Intake and Developmental history form for children (they just added one for adolescents that are supposed to be modified and used for children as well, but it is not remotely applicable and does not have questions about developmental history, etc.). I have one I send out separately prior to all child sessions and have submitted it for SP to use without success. Likewise for a Consent for Treatment of a Minor form as well as a Might anyone has an interest in making any of these forms into a template? SP has no plans to do so, unfortunately. If so, I can pass on some forms that I am very happy with. Otherwise, any suggestions as to how we can push the folks at SP to add it/them?

Hi,
I am also new to SP but I custom made Adolescent Intake. It was very easy. You start with clicking New Templates ( through Account---Setting-----Notes and Document)
You just write the question in question box adjust short or long answer and click save.
Then hit add new to add other questions. To start with I just select long Answer option. You can add developmental history if you want.
It can be previewed.

While it is understood that we can create them ourselves, the issue is one of time that each of us would have to devote to creating a new template (my child intake form is more than 10 pages) and many of us do not have that 'extra' time to devote to it. It would make infinitely more sense for SP to create a template that we can ALL use for children, as we are paying for the program and providing the feedback that this remains 'lacking' after all of this time. And though it is easier/shorter, the fact that there is no Consent for Treatment of a Minor Form is also quite bewildering. Granted, the option to upload our own forms back into the portal after printing them out is an improvement, but some folks really struggle with understanding and/or following through with this part. Simple Practice, can you please help us understand this apparent 'gap' given the many of us that see children?

You can find an Adolescent Intake Questionnaire in our Template Library under My Account > Settings > Notes and Forms > Template Library.

Ayelette--yes, we are asking for a CHILD form in addition to the Adolescent form WITHOUT having to create ourselves. Still don't get why SP can add the latter but not the former! It would make much more sense to have one that we can all use vs. having every SP provider create their own template. And, no, the Adolescent Form is not the same and they clearly need different questions, etc.

We consider customer input for new forms and have added suggested customer created templates in the past, however we are not able to create new forms on demand for all customers.
Some resources that may be helpful are:
- Customizing our ABA Child Intake Form in our Template Library.
- Adding your own consent form under My Account > Settings > Client Portal > Shared Documents and Files > clicking +Add Consent Document > assigning a title > adding in as much text as you need > clicking Save. These forms will automatically include an eSignature field when you share them with a client.

Hi Marie, in the last year, we've added several Intake Forms specific to minor clients. You can find them in Settings > Notes & Forms > Template Library. Use the dropdown menu at the top of the page to "View All Professions." We have an Adolescent Intake Form, an ABA Child Intake Form, an Occupational Therapy Intake Form, and more. If you can't find the exact template you're looking for, use the +Add link to add it to My Notes & Forms, click on the form, then edit it.
