Clinical Supervision Template

I created a clinical supervision note in the template today.  I put my therapist in as a "client" and used her email.  I figured this was the best way to create a folder for her documentation.  I hope that this will work.  I copied the link for the preview.  Any other suggestions as to keep documentation for supervision simple would be greatly appreciated.
