I use a credit card authorization form that is very specific and they sign it online along with all the other intake forms. I thought it was a Simple Practice form but it’s been a while so I forget. It’s great to have! I am on cell phone and haven’t used this community forum yet so not sure how to share with you.

Hi Caroline, you may want to check out our Note and Form Template Exchange at community.simplepractice.com for a credit card/AutoPay authorization form to borrow. While we strive to make sure that all of our Consent documents are solid and our legal team has gone over them, we are not legal entities and as such you would want to check with the regulatory bodies in your state for your specific practice needs.

Hi Gillian, I think that is where I am already posting this inquiry and didn't see anything prior to my post.

Tara J. if you ever have access to your form through a laptop, I'd love to see it. Thanks!

Mine is similar to this and includes a reminder that card will be charged according to my cancellation policy...

    Tara,

Thank you so much for these. That is incredibly helpful and kind of you to take the time to send them.

How can we include the above form in our intake packet that we send to clients to complete on client portal? I have tried the upload option but clients are unable to fill the necessary information as forms are not fillable. Any advice would be greatly appreciated.

You would have to create your own based on the wording of these other forms as examples. It would be VERY helpful if SP had a template already in place, but they don't.
