Getting authorization for Autopay
AnsweredIt seems as though it would be legal requirement for clients to fill out and sign a form enabling their credit card to be charged using Autopay. I would like to use this service, but feel uncomfortable automatically signing them up or only receiving verbal consent. Is this a question that has come up before? Does anyone have a template for such an authorization form? I think SP should have one.

I use a credit card authorization form that is very specific and they sign it online along with all the other intake forms. I thought it was a Simple Practice form but it’s been a while so I forget. It’s great to have! I am on cell phone and haven’t used this community forum yet so not sure how to share with you.

While we strive to make sure that all of our Consent documents are solid and our legal team has gone over them, we are not legal entities and as such you would want to check with the regulatory bodies in your state for your specific practice needs.

Mine is similar to this and includes a reminder that card will be charged according to my cancellation policy...
