  Thank you so much Mary! So until it becomes a template I guess we have to fill out by hand and scan into file? Is there another way?

  So is this something you send to your clients through the portal to fill out before they come in?

  Tammy - yes, I've also made a form that I send to the parents ahead of time. So I have two forms - one that the parent fills out in the portal prior to the appointment, and then I complete this Intake/Eval (progress note) during the first session.

  Thank you...I see the form...the link...I can't figure out how to get it into my "notes and forms" to be able to select it for use with clients...either for them to complete or for me to do with them.  I'm sure I'm missing something or just haven't learned a trick yet...I know how to get it from my own saved documents in my computer to a clients portal.  Just don't know how I can get it so I can see it in that list of notes and forms.

  Susanne S - there’s no way for you to upload it. SP has to decide which of the forms posted here they make available. I think you can “vote” to have it added or something? It’s a bit of work - but you’d have to re-create this form using the form builder.

  Okay...thank you so much.  So I'm not missing anything! : )  I wish they would have a way for us to add to the portal as an option for everyone.  I will try to find out how to vote.
