Medicaid Compliant Templates

I am seeking Medicaid compliant template for (1)care plan, (2 )progress note, and (3)initial evaluation/first session. I believe the care plan and initial eval needs to be signed - which is discussed in a separate post but wanted to bring up that would need to be included in the template.

Have you ever received any feedback on your request? I have recently had several submitted medicaid claims in Texas denied due to insufficient documentation according to their General Medical Record Documentation Requirements.
If I knew exactly what medicaid is looking for and could include that in my template, it would save a great deal of hassle and heartache.
