Hi all,

I recently did an intake with a client who later requested some support around their short-term disability claim. His work situation was stressful to the point of causing a GI concern with a diagnosis from his PCP. I later received request from his disability office for my intake note for the ct. My inclination is not to provide the intake not but a letter supporting his claim that the work environment was contributing to his medical diagnosis as well as significant stress.

I would love to hear any feedback or suggestions. Thank you!
