How to have clients sign treatment plans?

In the state of Arizona, it is required per the Board that all clients sign off on their treatment plans and then reviewed and signed again at a minimum of 1 year. In SimplePractice how can we have clients sign treatment plans? We are really freaking out on how to do this as this could be a game changer for us in switching services if we cannot have them sign them. Thanks for any help.

    Hi Tony, there are a few different ways that our customers get signatures on treatment plans. One option is to print the treatment plan, have the client physically sign it, then scan it back into the SimplePractice file. Another is to add a Consent Form that can be sent through the Client Portal called a Consent to Treatment Plan form. This can be sent directly through the Portal for signature for a paperless experience for your client.

    Hi Tamara, SimplePractice does not currently provide a Consent to Treatment Plan form. I'd recommend checking with your legal and regulatory authorities to confirm that this process would work for your practice, and for sample forms or ideas for wording this form.

    Hi

    I am a mostly paper free practice and want to continue to waste as much less paper as possible. I am interested in buying a signature pad to get signatures from clients. I am concerned about the quality and compatibility of different products. In the past, I did not have any luck with Topaz. I would like a product that is practical and easy to install. Can you suggest any product?

    Hi Jorge, the SimplePractice team doesn't have any specific recommendations for outside hardware for use with SimplePractice.

    Can you all make an update to the platform that allows for signatures of treatment plans? Every practice for every state has to have a signature if they take insurance.

    I would like to second this as an ongoing problem, especially in the wake of covid-19. I need a way to have my clients sign their individualized, personalized treatment plans, at least every 6 months as this is a requirement for Medicaid.

    Is there a way to simply share a client's treatment plan via the portal, without having to download it to my computer and then send through messaging? I can't find how to share the treatment plan once it is completed.

    Just want to be clear, there is still no way to download an outside document to our clients portal and have them be able to sign electronically??

    Hi Kandi, you will need to download the Treatment Plan as a PDF first, then upload it to the Stored Documents section of your client's chart. Then, you can share it through the Share button.     It would be great if you could give a direct answer about how long it will be until we have access to treatment plan that can be electronically signed.

    2. ability for therapist to use that template to complete it individually for each client, sign it themselves and date it, and save it. And send it to client via the portal.

    This is a critically important step. Clinicians need to be able to send a completed treatment plan, one that has been reviewed with the client during their initial telehealth session, and add a way to obtain an electronic signature from the client on this treatment plan. Thanks!

    Any update on this - we need to be able to have clients sign treatment plans electronically this is SOOOO important and required for audits.  Why is it so easy to add to other forms but not the treatment plan forms???

    Is there a way to send a specific document to a patient and have them sign it?  I know you can put initial documents for the patient to sign when they are a new patient to the practice however, we have new patient forms for different type of patients.  Is there a way we and pick and choose what documents we want to initially send the patient?  If so how do we do it?

    Tammy:  You can make a separate document and save it, forward to the client to sign.  It gets saved in the Portal Document area.  My MAJOR concern is that I save it there forward it, pending the patient signing it will sit there.    This is a work around you can use for now.

    Unfortunately another issue with SP is there is no way on our part to separate those docs out.  Ex. I would like to be able to separate out - Counseling consents, reunification consent, Treatment plans pending to be signed.  Adult consents, Children Consents

    I worry in a group practice that an admin or therapist will forward the treatment plan I saved in there in error when they are clicking on forms to share.  Then a HIPAA VIO issue occurs,  This is why it is imperative issue for SP to fix. 

    As a group I would like the ability to make an area for each kind of consent, each therapist, etc.  This is a reason why the treatment plan should ONLY be in the client file, not sitting in client portal consent area.

    This is not a practical process. Simple Practice should have come up with something by now. This concern is several months old.

    I would like to suggest that Simple practice create the ability to send a PDF securely through the portal, to be signed by another person. I'd pay extra for this!!

    It is necessary for me to obtain client signatures on their treatment plans every 6 months, as required by medicaid, in order to be able to provide services. It would be extremely helpful to just have this added to the treatment plan. I'm not willing to pay more for it, but it seems like something that many many many of us need!
