Here are preview links for Gottman Method forms for couple counseling. I'll be continuing to add to this list, so make sure to follow this post if you want to see those other forms.

There's no way to add it directly. You can only do two things with a template you like.
1. Upvote it by clicking the heart at the top of the post next to my name. Simple Practice says they regularly add the templates with the highest number of upvotes to the system, but not sure how regularly they do this, as I haven't seen a new template in quite some time.
2. Use my template as an example and create it for yourself in your SP system.
It would be phenomenal if SP provided a way for us to add the templates we like to our own systems at the click of a button, but not how it works for now. -
Katherine: Disclaimer: I AM NOT A CERTIFIED GOTTMAN THERAPIST. I use Gottman methods and have taken the first and second levels.
That being said, this is how I set up my couples. I do the assessment over a period of four sessions. "Oral History" are the questions for the first session with the couple. Then I have them do the Gottman Check Up Online before the fourth session. The second and third sessions are the break out sessions where I meet with each partner individually, the "Individual Interview Forms" are used in this session. I review the results of the Check-Up before the fourth session, where I provide feedback regarding findings from the first three sessions and from the Check-Up. In that fourth session, we address their strengths, areas of focus for treatment, and develop goals. Hopefully, this is helpful!
