Client intake forms through portal

    I have multiple intake forms that I need to have patients complete, but it appears that I am only able to send one per email? Is there a way to change this? I do not want to combine all questionnaires into one, and I also don't want to inundate my patients with multiple emails.

    Hi Lynn, I'd be happy to help. If you're not able to select an intake form to send to a client, it's likely that the intake form has already been sent, but not filled out. You can check to see what has been sent to the client's Portal under the Shared With Client section on the client's chart.

    I just created a new form to share with a client but even when I select it, it wont go into my client portal for sending to clients. It is an assessment questionnaire.

    The Safety Plan under Assessments is checked off in my Portal but it is not appearing under my Forms so I cannot send it to my client.

    The Safety Plan does not appear in my Shared Documents & Files even though it is checked off an one of the forms in my Notes & Forms. Therefore I cannot send it to my client.

    Hi Ana, I'd be happy to help. Under Settings > Notes and Forms, open the Safety Plan, and change it from an Assessment to an Intake Form. That will allow you to send the form through the Share button.

    Hi Ana, to send a new Intake Questionnaire to a client, go to the Share button on the client's Overview page and select the new form. Let me know if you're still having trouble getting this form sent to your clients - we're here to help.
