    Patrick, great job, I love these useful tools/assessments that clients complete in the portal and upon completion become part of their "chart."

    this will be a great form to add. Any assessments we can add will are always helpful since insurance companies like to see results and assessments are a great way to show this.

    Hello! I assumed that using the Templates forum was a way to get it added with enough votes. So this is the very reason I posted it here. With enough votes, in this same forum, I was able to get an ADHD Self Report measure added. I’m concerned with starting all over again with that forum. Clarity as to which place to use is not clear. Actually I thought this forum was appropriate.
