    The same is happening to me. I know I have the correct ID number and format. This appears to be a clearinghouse rejection, and with clients that have previous claims submitted with the same ID number that had no problems. What is up?

    Hello

    I am having the same issue - medicare claim being rejected even though the ID is correct ( medicare tells me it is a front end rejection - ie   simple practice is rejecting it - as medicare has no record of the claim.

    I have found that some of our patients have been issued a new Medicare number. Some of them were not even aware of the change. I would confirm the ID number with Medicare first.

    yes I am now using the new MBI # and verified it with medicare because that is being denied as well.

    it appears I am having the same issue as the other people.  I was submitting the claims last year no problem, but resubmitting the same patient since 6/30 are getting denied with this 164 code.  Please give any advice
