    Yes, PLEASE make a way to set ALL of the fields (answers) on the Demographic Form to be REQUIRED.  This would save so much time so I don't need to contact each client to get necessary information to bill insurance.

    Yes, please allow us to make these fields required. I often don't realize when they're not completed, and then I don't have the info I need to file an insurance claim. At minimum, that information should be required.

    Can the client address be a required field for the demographics?

    It would also be great if there was a place on the demographic form for people to put who they want their Superbills to be made out to. Especially for people who works with kids and teens.

    Hi all, apologies for our delayed response - our Success team reached out to Karen privately to assist, and it looks like we missed responding to this post. There's not currently a way to set the fields on the Demographic Form to be required, but we're always welcoming suggestions for SimplePractice on our Ideas board at simplepractice.uservoice.com.

    I'm trying to find out if there is a required place for the client to put a social security number. I have found out that "collections" needs a social security number to collect bad debts. Thank you, cheri r.

    As Gillian described, if you have a moment, I definitely encourage you to vote or comment on this Make certain demographic information mandatory idea on our Ideas & Suggestions board. Our product team regularly reviews the ideas on this board as part of our development process.

    Our Demographic Information form does not currently request a client's social security number. However, you can create your own customized intake form requesting this information and requiring that this question be answered.     In creating any form, it would be a great advantage to use the same "Column Header" in our questions that you use in you database. Is there a way for you to provide us with these words or could you design these forms that only use your database "Column Headers"

    Hi Dr. Cuva, you can add Section Headers to forms you create through SimplePractice, or any forms you'd like to customize in SimplePractice. Go to Settings > Notes & Forms and click on an existing form to edit it, or click +Create New Template. Next, click the Add New button at the bottom of the editable template. When selecting a question type, you can choose the Section Header option. 

    LaVerne, currently there's no way to edit the Demographics Form, which includes the ability to make a form field required. The reason for this is that the Demographics Form directly populates into a client's file when it's filled out, so no adjustments can be made. You can always create a custom template in Settings > Notes & Forms > +Create New Template and require an answer for any of the questions you add. Although, any form you create won't populate into the client's file the way the Demographics Form would.
