    Hi Megan, happy to help. SimplePractice has a built-in ROI template that you can customize, and it includes an eSignature checkbox. To find the template, you can visit My Account > Settings > Notes and Forms

    hi, when i fill out the ROI there is no place to save it. it only has a button for print, and when i hit the "back" button it clears the whole document.

    hi, thanks for your response. however, i still don't see it. i am sure it's me, maybe you can walk me through the process? i click on forms, and find my ROI-copy. i fill it out, but it only tells me that i am "previewing my ROI, this is how it will appear in your portal" and only allows for printing. what am i doing wrong? i only see a view, duplicate, or trash option. HELP! thanks!

    Hi Tammy, happy to help. You can edit the ROI template under My Account > Settings > Notes and Forms. However, SimplePractice doesn't currently have a feature that will autofill the client's responses. You may want to consider changing some of the Form Element types on the ROI template to "Paragraph text," then fill in the form, so that clients submit their signed ROIs with the correct information.

    Joanne-- was having the same issue... then noticed that the default is that ROI is saved under progress notes-- when you go to view an ROI there is a dropdown menu where you can select Intake Forms. I just made that selection and was able to then send the tailored form (had already done that) to my client as an intake form. Not sure why that is not the default--since the form will need to be sent, esigned and returned by client-- but that is how it is set up. hope this helps.

    I am having the same problem. I complete the ROI but there is no way of saving/sending the completed for to  the client for a signature. I do not see a save button at the bottom of the form.

    Ugh, same issue as a few of you.. I don't understand why there is not a ROI template in the client's chart that we can customize and send out to them as needed? Am I missing something? I have just been printing the template, manually completing it, scanning it back, and then sending to client. It seems like that is way more tedious than it should be, as many other SP features are much easier.

    Ashita, I ran into this issue when I needed a client to sign an ROI. I had to send them a blank one and they had to choose the options which is stupid. We need to be able to complete the document and have the client sign but it doesn't seem to work that way. You can send them an email link through SP to complete the form but it's blank for them so they have to fill it out themselves.

    Same question When I send a very generic black ROI many clients do not know how to complete it.  I want to fill in the information and send it to the client to e-sign.  Is that possible?  Do I have to open the template and customize it each time?

    Lena I tried changing the form to make it e-signable but it can't be if you move it out of Intake. The setup is so frustrating because there is no way to get an signature  and complete the form for the client without having to print, scan, all that stuff....

    Once I figured out the trick it was relatively simple to do this.  There are instructions about halfway down this page for saving the ROI template as an assessment.  After you have done that from the client's overview page you can fill out the form, share it with the client and the client will get an invite to sign it in the client portal.  https://support.simplepractice.com/hc/en-us/articles/360041722112-Adding-assessments-diagnoses-and-treatment-plans
