Intake Forms vs Consent documents
AnsweredI am confused as to which option to choose for any given form? For example, Electronic Communication Policy -- Intake Forms section? Or Consent (Client Portal) section?
I have several like this. It is unclear where they belong on the platform.

Hi Josh, happy to help. Read-and-sign Consent Forms should go under My Account > Settings > Client Portal > Shared Documents and Files > + Consent Forms, downloadable PDFs can go in the same place, and fillable Questionnaires and Assessments can be added and edited under My Account > Settings > Notes and Forms.

Hi Kate, to upload a form for clients to read and sign, go to My Account > Settings > Client Portal > Shared Documents and Files and click +Consent Form. Then, copy in the text that you'd like to include, and click Save. You'll then be able to share this new Consent Form with clients through the Share button on their Overview page.

Hi Mallory, no there isn't. In order for your client to electronically sign a document in SimplePractice, it needs to be recreated as one of our templates. You can copy and paste the contents of your PDF document in Settings > Client Portal > Shared Documents and Files > + Consent Document. If you attach the PDF Document to the Read-Only Files section, you can send the document to your client through the Client Portal and they'll be able to view and download it, but they won't be able to sign it electronically.
