    Does anyone know exactly what date goes in the Affective date section of the Notice of Privacy Practices? Is it the day you started the private practice? or the date they sign the consent?

    Have you been able to create this as a template in Simple Practice, or do you send this form separately to clients? When I have tried to create a template, the format lumps everything together in paragraph form and doesn't let me have bullet points or separate lines for text. I appreciate your help!

    Hi Jennifer,

    You are correct, you will have to format the template in Simple Practice, as the template I created has been formatted using lots of section breaks, line breaks, and bullet points.  The purpose of the document is to give you the text, but you will have to adjust it to make it look nice for your clients.

    Hi Ryan,

    Thank you for your quick response.  For some reason, I am having trouble formatting documents in Simple Practice.  I tried with my own forms, and I tried yours (thank you so much for supplying yours), and it lumps everything together as one paragraph. I have contacted customer support to see if there is a way to fix this issue. It seems like there should be a way to type forms using this same formatting system where you can use bullet points, bold font etc.

    Hi Jennifer,

    Unfortunately, the editor available for creating templates is very limited in terms of formatting options.  You can't embolden fonts or create bullet points (I just used asterisks, or you can use alt-key codes), like you can when editing progress notes.  You just have to get out-of-the-box creative.  I had the same problem when I copied and pasted my NPP document into the template editor.  Then I had to break up sentences using line breaks in order to get a more readable appearance.  It's a bit of a hassle, but once done, it's real easy to dispatch those forms out to clients using the client portal.
